Below you will find the application for a CAUSE Foundation\u00a0grant.\u00a0 These funds are intended to be a short-term<\/i> bridge to help United Airlines Flight Attendants and their dependents in times of illness, injury, or disability.\u00a0 Our objective is to help with the basic necessities through an unexpected difficulty, or to provide breathing room while more permanent arrangements are made in a long-term situation. We can assist with excess medical bills but cannot pay credit card debt or other personal financial obligations.<\/p>\r\n\r\n\r\n\r\n

We realize this may be a difficult process for you and want to make it as easy as possible.\u00a0 Before you begin this application, it is important that you call A CAUSE Case Worker.<\/span>\u00a0<\/em><\/strong><\/p>\r\n\r\n\r\n\r\n

Once all the information has been received, our policy requires that two CAUSE Case Workers review your application to determine the amount granted. The amounts are decided on a case-by-case basis according to need and the availability of funds. The number of applicants requesting assistance can impact that amount and ongoing assistance may vary monthly due to the volume of requests.<\/p>\r\n\r\n\r\n\r\n

The funds received from the CAUSE Foundation are grants. They are not taxable, nor do we ask that they be paid back.\u00a0 This is money our flying partners have donated to help us help each other. We have an obligation to spend it as wisely and carefully as possible.<\/p>\r\n\r\n\r\n\r\n

We want to assure you that this process is confidential.\u00a0 The Health Insurance Portability and Privacy Act requires your written permission for any information about your situation to be discussed outside the Foundation.<\/p>\r\n\r\n\r\n\r\n

We are here to help, and we promise to assist you as quickly as possible.\u00a0 Please understand, though, that this process can take several weeks to complete.\u00a0 Every CAUSE Case Worker is a volunteer and they have other obligations. Please treat all our volunteers with respect. Our schedules may prohibit us from getting back to you as quickly as we would like.\u00a0 Feel free to contact us at any time, and we will return your call or email as soon as we can.<\/p>\r\n\r\n\r\n\r\n

  1. Most recent Medical note or Absence Certificate (must have a doctor\u2019s signature) FMLA paperwork is NOT<\/strong><\/span>\u00a0<\/strong><\/span>acceptable.<\/strong><\/span><\/span><\/li>\r\n\r\n\r\n\r\n
  2. Copies of pay stubs from the last 2 months flown (1st and 16th) 4 pay stubs total<\/li>\r\n\r\n\r\n\r\n
  3. If applicable, spouses pay stubs from the last 2 months<\/li>\r\n\r\n\r\n\r\n
  4. All bank statements from the previous 2 months and up to current date of application. All pages, even if the last page is blank. If your statement is 6 pages, we need all 6 pages. \u00a0Updated bank statements will be requested every 2 months.<\/li>\r\n\r\n\r\n\r\n
  5. Latest 401K statement<\/li>\r\n\r\n\r\n\r\n
  6. Federal Tax returns (W2\u2019s are NOT tax returns and will NOT be accepted!) from the previous 2 years\u2026JUST THE FIRST 2 PAGES OF YOUR ACTUAL RETURN THAT SHOWS INCOME AND DEPENDANTS!<\/li>\r\n\r\n\r\n\r\n
  7. Copy of your sick leave balance from your Pay Register in CCS.<\/li>\r\n
  8. \r\n

    If you are receiving Workmen\u2019s Compensation from an OJI, a copy of the award letter from Sedgwick showing the weekly benefit.<\/p>\r\n
